It would be cool if Italians were split into three/four new civs, republic of venice, republic of genoa, papal states or milanese, just like indians had a dlc like this.

We already have three Italian civs.
This isn’t age of city-states. Italians represent them all very well.
This isn’t like Indians.
That civ was split because it covered an area and population nearly the size of Europe, with multiple wildly different and unrelated cultures and ethnic groups.
If you want more Italian civs, then everywhere else needs a LOT more civs first.

Go make a Scenario in the editor. There is so many triggers these days to adjust the game and shape new civilizations.
If you need an example, i made my own scenario of the greater netherlands.
You can do the same for Italy or whatever map you like 
There is also modpacks with that custom civ website as an option.

This request frightened me.
If did so, people will ask “Why there is no Tuscany/Florence/Genoa…”, and this game went to Age of City States.

I love the idea and this has a lot of potential.But will it add anything groundbreaking to the game is questionable.
I just have a new idea: each time when a player choose to play Italy, it will randomly became a subculture of it and each subculture get their own buff/UU/UB.
Easier option is to give it an option to select the city state similar to athenians can can shift between ways of government.
People will not rest until every single last tiny village in Europe has a civ dedicated to them.

An Italian Wars DLC would be a great opportunity to introduce more civs with the currently underused Mediterranean architecture set. Plus did you know that only five of the existing wonders are in Italy? Even the whole of Africa and America combined have more than that! /s
On a serious note, the OP should try Age of Chivalry: Hegemony – it has separate civs for Florence, Genoa, Milan, Naples, the Papal States and Venice.

Considering the several modern fantasy RPG civs suggestions you come across around here, that’s not a bad idea at all.
Meanwhile in Africa:
Keep in mind this is only the 900s so some like Vandals are missing as well as Swahili, Yoruba and others… could be here for like 4 expansions and still not be done
